•Please note- my insurance doesn’t allow me to use numbing cream.

• If you are booking for a group, please book on separate form but tell me that you are booking for the same day!

• If there’s multiple tattoos please put a description for all of them with sizing and placement.

•Please pay the booking fee before confirming your appointment- you will not have an appointment until this is paid. It will be in your notes.

• I will send a precare sheet with all information needed on. These will be my polices, where to park and how to pay. Please include medication and medical conditions as I may send you away on the day.

A consent form will be filled out in the day too.
